| Center Location & Hours | The Trauma and
Anxiety Disorders Clinic is located within the
Psychological Training and Consultation Center, which is on the second
floor of the Health Professions Building on the campus of Central Michigan University (see
map). General Clinic hours are from 9 a.m. to 6 p.m. Monday
through Thursday and from 9 a.m. to 5 p.m. on Fridays. Evening
appointments, from 6 p.m. to 9 p.m., are arranged on an individual
basis.

| Cost | The fee for an
initial 3 hour evaluation is on a sliding scale, from $15 to $180, based on the
client's household income and number of dependents. The fee for the treatment
varies depending on the length of treatment. The fee schedule is
designed to allow anyone to participate and no one will be turned away
for financial reasons. In addition, our research programs may offer some
treatments free of charge.

| Cancellation Policy | Except in special circumstances and cases of emergency, clinic
appointments must be cancelled at least 24 hours in advance to avoid
being charged for the missed appointment.
