Notes for Spring of 2006:
If the abbreviation "RM" is found, it refers to a book that is no longer in use.  These references are being edited out.
As of January 2006, sections VI and VII have not yet been updated to reflect the use of the new book, Meditations of a Tantric Abbot.  They refer to "IT", a book no longer in use.
