Please return this form with your registration materials/payment or separately as early as March 10.  Time slots will be filled as of that date.  Reservations will be held until March 31 for those who send in this form in advance of registration.

Please inform us if you have to scratch your entry, so that your time can be given to someone else.

Registrants will be informed of their times after the March 31 deadline.
